Student, Medical and Ethnicity Information Form
  • Please fill in the details below as fully as possible.  Not all of the information is mandatory, but it will assist the school in communicating with you and will also support your child’s learning.

    Please note the Data Protection Act 2018 allows individuals to withdraw their consent at any time.  If you wish to withdraw your consent for all or some of the items which you previously consented to, please contact the school.

  • If your child will be travelling to school by bike, a ‘Cycling to School Agreement’ form must be completed.  Please print out and complete the form by following the link below.

The following information is important to us and will be treated with the strictest confidence as the school receives funding for these categories
Lockers
  • A locker can be provided for a student at £10 per year. These are based either in the student's tutor group classroom or just outside in the corridor.  We recommend that a padlock be purchased once a locker is paid for and allocated.  (More information about payment of a locker will follow).

  • Please note, if you have indicated above that your child suffers from a medical condition which needs medical intervention, for example an asthma pump or Epipen, a separate Medical Plan form will be sent to you for completion.

Ethnicity Information
  • Please note the information in this section is mandatory.  Any information you provide will be used solely to compile statistics, which will help us to support all our students in the most effective way.  The statistics will be passed on to the Local authority and the Department for Education.

    Our ethnic background describes how we think of ourselves; it is not the same as nationality or country of birth.

    The Department for Education recommends that young people aged over 11 years old should have the opportunity to decide their own ethnic identity.  Parents or carers are asked to support and advise children aged over 11 in making this decision.  Students aged 16 or over can make this decision for themselves.
